REQUEST YOUR MEDICAL RECORDS
To obtain a copy of your medical records from Spanish Peaks Regional Health Center or our clinics, please select the appropriate Release of Information form from the options below. Please complete all sections of the form and return according to the following insutrctions.
Completed Release of Information forms may be faxed, mailed, or brought into the hospital's Health Information Management Department at:

Please specify if you would like to pick up the medical records in person, have them faxed to you, or mailed to you.

Requests for medical records are processed in the order that they are received. Medical records will be delivered within 14 days upon receipt of the request.
